Social Work
Our Social Work Team is part of Norfolk County Council Adult Social Services Department. It is fully integrated into the multi-disciplinary team and based at the Colman Centre.
The team works with both inpatients and those day patients who do not already have an active Social Worker.
Services include:
- Support the multidisciplinary team to in-reach into acute services prior to arrival at the Colman Centre
- Assessment and planning with patients and carers for discharge.
- Support to partner, relative or friend and providing a carer’s needs assessment.
- Creating and reviewing the appropriate care package
- Exploring and arranging appropriate care packages, e.g. assistance to support patient returning to live at home or a short-term or permanent residential care home placement.
- Advising on Social Services eligibility criteria and charging policy.
- Sign posting to and liaising with other agencies, e.g. Housing Department and Benefits Agency.
- Organising parking concessions for disabled drivers or passengers through the Blue Badge scheme.
- Providing information about relevant services in patient’s local area, e.g. social and leisure activities, transport services, meal delivery services, RADAR keys for disabled public toilets.
- Working in conjunction with the multi-disciplinary team to address access and housing needs
- Co-ordinating the ‘Continuing Care’ and ‘Single Assessment’ process
- Making assessments under the Mental Capacity Act and responding to the provisions for patients under this act
- Providing a service in accordance with ASSD policy and procedures.
All services reflect appropriate health guidelines and policies.
