In-patient Nursing

The in-patient nursing team provide comprehensive specialist nursing care to patients requiring rehabilitation who have complex health and social needs.  The nursing team work with multidisciplinary colleagues to support the patients from before admission, through in-patient care to an effective community discharge.

We do this by:

• Using a primary nursing approach to ensure continuity of care and formation of effective relationships with the patient and their family

• A thorough and rigorous approach to nursing assessment, care planning and provision of nursing care

• Working as integrated members of the multidisciplinary team

• Supporting patients to achieve their rehabilitation goals

• Creating a welcoming, therapeutic and friendly environment conducive to healing

• Creating and maintaining good working links with associated services

• Working with the multidisciplinary team to achieve effective and successful discharge to the community

• Providing care and support around the clock
