Day Patient Services
The Day Patient Service is based on two sites in Norfolk, Colman Hospital, Norwich and St. Michael’s Hospital in Aylsham.
St. Michael’s service takes place on Mondays and Thursdays each week. The Colman Centre service takes place in Jubilee house five days a week.
Referral to the service is by written letter giving as much information as possible and directed to either consultant or to the coordinator of the service. New patients receive two initial appointments, one with the multidisciplinary team and the second with a Consultant in Rehabilitation Medicine. Each patient is assigned their own key-worker and enjoys regular case conferences to review their therapy and progress. The patient can invite their partner, family or other professionals to these case conferences as they see fit. These meetings are held every 3 months and patients are encouraged to identify and agree goals and targets during the sessions. The case conference system works in harmony with the therapy and medical reviews which may be held at three, four, six or twelve monthly intervals depending on the issues identified.
An informal carers group takes place on a monthly basis. Some months, invited guests speak to and inform the group of relevant issues identified by the carers. Some family members or carers may need to be seen on an individual basis for support and advice at this very emotional and psychological time.
The Service has a Head Injury Coordinator coordinator, patients referred to the Colman Centre from acute hospitals are assessed by the Head Injury Coordinator and family members may also be seen.
We have four community rehabilitation assistants who work in the hospital setting and the community. The rehabilitation assistants outreach work helps patients with their self confidence and self belief in order to ease them back into their life in the local community.
